Are You In-Network?

We accept and are in-network with many of the major insurance companies. It is the patient’s responsibility to verify their own physical therapy benefits. As of January 1, 2023 we are no longer in-network with Aetna. Please call the number on the back of your card to ensure that Choice City Physical Therapy is in-network for your particular plan. We will do our best to verify coverage once we have a copy of your insurance card.
